Good Faith Estimate (GFE)

Attention all uninsured patients or patients who are not using insurance for our services:

While we provide the costs of service up front, over the phone, to all of our patients when they first schedule an appointment with us, you also have the right to receive a written “Good Faith Estimate” of your mental health expenses at Living Hope Counseling Services under the No Surprises Act implemented in January of 2022.

Under 2799B-6 of the Public Health Services Act, upon request, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

-You have the right to request and receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like psychotherapy and psychological testing.

-We will provide a Good Faith Estimate within 1-3 business days of scheduling your appointment depending on the date of your appointment. You can request a Good Faith Estimate before you schedule a service.

-If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

-A copy of your Good Faith Estimate can be found in your client portal.

For questions or more information about your right to a Good Faith Estimate, or how to dispute a bill, visit www.cms.gov/nosurprises or call Living Hope Counseling Services at 513-371-5135
